Attestation Statements

I have reviewed the Code of Conduct document and agree to the terms. I am committed to upholding the highest standards of behavior for myself, my child, and all individuals associated with me. I understand that any inappropriate conduct will be reviewed by the FRYA Board, and I agree to accept and respect the board’s decisions regarding such matters.

I, the Parent/Guardian, hereby give consent for any and all medical attention to be administered to my child in the event of accident, injury, sickness, etc., under the direction of an FRYA representative, until such time as I may be contacted.

I do hereby waive, release and hold harmless the Flat Rock Youth Association, its officers, coaches, sponsors, supervisors and representatives for any injury that may be suffered by my child in the normal course of participation in the designated sport above and the activities incidental thereto, whether the result of negligence or any other cause.
